1. Initial Conversation

conversation. An initial conversation will help us get to
know more about you and your loved ones needs.
During this conversation we will take the time to listen
to your needs and understand your circumstances.
2. Home Visit
Once we have got to know you a little better, We will
organise a home visit, where we can further discuss
their needs. Our experienced care management team
will thoroughly review your loved ones care needs and
produce a detailed care plan, suited to their needs.
we will allocate the best care professionals who are a great
match to your loved ones personality, we match our highly
skilled care professionals to our service users by matching
characters , hobbies and interests. This encourages a good
environment for clients as people tend to build better relationships when there is a shared interest.
3. Meeting Your Care Team
Once we have selected the right support team, We will arrange a meeting where we will introduce your loved one to their Care professional team. We will go over the care plans and certify that it meets all your care needs, also meeting the criteria set out by other health professionals. we start to introduce chosen carers to you to start building relationships and helping to ease the transition as quickly and stress free as possible.
